Intratunnel Phacofracture Technique of the Manual Small Incision Cataract Surgery

Image
Intratunnel Phacofracture Technique of the Manual Small Incision Cataract Surgery Intratunnel phacofracture technique is a new technique of manual small incision cataract surgery (MSICS).In this technique, the nucleus is broken inside the 4 to 6 mm wide sclero-corneal tunnel with the help of the Lewis lens loop. MSICS is significantly faster, less expensive, and less technology-dependent than phacoemulsification and has been extensively practiced in developing countries such as India. However, the drawback of the most commonly practiced MSICS techniques Blumenthal, visco-expression, irrigating wire vectis, and fish-hook needle is that they all require a large incision of 7 to 9 mm, which leads to induced astigmatism. The  Intratunnel Phacofracture technique  was innovated by Dr. Sudhir  Singh 1,2 Intratunnel Phacofracture: A New MSICS Technique (EyeWiki)   References 1.Singh S. (books2read.com)   Introduction Posterior polar cataracts (PPC) are cataracts lying on the posterior pole of the lens. They are stuck to the posterior capsule, so there are many times more chances of the post capsule ruptures and related sequelae. In this E Video book, I am going to share my approach. Pre-operative considerations and plans 1.      Patient Counseling: Patient counseling is very important and it requires substantial chair time with a patient and relative to explain the complexities of the disease, procedure, and chances of re-surgeries in writing. 2.      Anesthesia and Akinesia: My preferred choice is peribulbar local anesthesia with good akinesia. 3.      Capsulorhexis Size: I prefer moderate size centr...

(books2read.com) Intumescent white cataracts pose challenges in every step of cataract surgery. Capsulorhexis is the most challenging step followed by nucleus management. The capsulorhexis is prone to anterior capsular radial tear extension or also known as the Argentinean flag sign. The nucleus management by phacoemulsification becomes further risky in the presence of the Argentinean flag sign. In this article, we will discuss how to achieve intact capsulorhexis in maximum numbers of cases, if intact capsulorhexis is not achieved then how to proceed for safe nucleus management.
